Help us coordinate access to specialty care for underserved communities as we continue to expand from the San Francisco Bay Area to a 24-county region of Northern California (North Coast, North State, and part of the Central Valley).
We are seeking people to join our diverse team who are passionate about health equity, have experience in our expansion counties, and have skills in a combination of the following areas:
Multilingual culturally responsive health care coordination in Spanish and English, additional languages a plus
Health care coordination and/or coalition-building among community clinics, physicians, hospital systems, and/or community-based organizations to develop services for immigrant and disadvantaged populations
Team learning and development leadership, with a focus on the promotion of diversity, equity, and inclusion
Collaborate in developing communications materials and messages
